A respected healthcare organisation is seeking a skilled and compassionate Senior Nurse Assessor to join its specialist team supporting individuals with complex health needs. This role involves coordinating assessments, managing care pathways and ensuring patients receive the right support in the most appropriate setting.
What You’ll Be Doing
In this role, you’ll be responsible for:
Conducting detailed health assessments for adults with long-term or complex conditions
Leading eligibility reviews for NHS-funded care, including Continuing Healthcare (CHC) and Funded Nursing Care (FNC)
Managing a caseload of patients and coordinating multi-agency care plans
Supporting appeals, fast-track applications and clinical panels
Advising care providers and families on care options and funding pathways
Ensuring compliance with national frameworks and safeguarding protocols
Collaborating with local authorities, hospitals and community teams
Contributing to service development, audits and clinical governance initiatives
Promoting patient-centred care and advocating for individuals’ health and wellbeing
Who You Are
You’re a registered nurse with a strong clinical background and a passion for improving outcomes for patients with complex needs. You’re confident in leading assessments, navigating funding frameworks and working across health and social care boundaries.
Essential Qualifications & Experience
Degree-level education or equivalent experience in a specialist clinical area
Current NMC registration
Experience conducting CHC/FNC assessments
Strong understanding of long-term conditions, palliative care and disability support
Skilled in multi-disciplinary collaboration and conflict resolution
Familiarity with safeguarding, clinical governance and NHS frameworks
Ability to manage caseloads, meet deadlines and work independently
Excellent written and verbal communication skills
Desirable
Master’s degree or post-registration specialist qualification
Knowledge of commissioning, DOLS and NHS funding structures
Experience working across health and social care systems
Understanding of SEND, Mental Health Act and Care Act assessments
