We work in partnership with a private healthcare to look for Clinical Nurse Specialist to join their Hospice. As the CNS, you will be involved in management tasks such as the development of policies, involvement in quality and audit initiatives and educational programmes for health care professionals, staff and students including involvement in helping to implement Care Pathways.



Requirement 

  • NMC Registration
  • Graduate-level or equivalent qualification in relevant discipline
  • Advanced communication skills training
  • Advanced clinical, managerial and leadership skills
  • Excellent communication and interpersonal skills
  • Able to work autonomously and as a member of a small team, as well as part of the wider multidisciplinary team
     

Duties 

  • Managing a caseload of patients with specialist palliative care needs in the community
  • Attending meetings and joint working with local primary health care teams and acute       hospital teams
  • Delivering education and training
  • Taking part in multidisciplinary meetings
  • Assess referrals appropriateness and urgency
  • Demonstrate skills in working with families facing loss and identify any particular risk factors
  • Promote and present the Hospice’s Vision, values and strategic objectives
  • Participate in teaching and training programmes
  • Identify, assess and meet the educational needs of the patient, family and carers
  • Participate in clinical audit
